If you are an heir of a deceased class member, please send copies of the following documents:

1. Death Certificate;

and

2. Last Will and Testament or Order from Probate Court establishing beneficiary(ies);

or

3. Statement showing the account was Joint Tenants with Rights of Survivorship account from the month of the decedent’s death (if applicable).

If a settlement check was issued in the name of the deceased class member, please return the check by mail.
